TFCC Injury (Triangular Fibrocartilage Complex)

What Is It?

The triangular fibrocartilage complex (TFCC) is a group of ligaments and cartilage structures that stabilize the ulnar side of the wrist.

Injury may occur from trauma such as a fall on an outstretched hand or from degenerative wear.

Symptoms

  • Ulnar-sided wrist pain, especially with gripping or rotation
  • Clicking, popping, or feelings of instability
  • Reduced grip strength
  • Swelling along the pinky side of the wrist

Diagnosis

  • Physical exam with pain reproduced during ulnar deviation or forearm rotation
  • TFCC compression testing
  • MRI or MR arthrogram to visualize tears or degeneration

Treatment

Conservative Options:

  • Rest and wrist splinting
  • NSAIDs for inflammation
  • Physical therapy to restore strength and control

Interventional Options:

  • Corticosteroid injections
  • Prolotherapy or PRP

Surgical Options:

  • Surgical repair for full-thickness tears or chronic instability
