Plantar Fasciitis
What Is It?
Plantar fasciitis is the inflammation or degeneration of the plantar fascia, a thick band of connective tissue that runs along the bottom of the foot from the heel to the toes. It is the most common cause of heel pain, often due to overuse, poor footwear, or biomechanical issues.
Symptoms
- Sharp pain in the bottom of the heel, especially with the first steps in the morning
- Pain that decreases with movement but worsens after prolonged standing or walking
- Tenderness along the medial heel
- Possible tightness in the Achilles tendon or calf
Diagnosis
- Clinical exam: Pain with palpation of the heel and dorsiflexion of the toes
- X-rays: May show heel spurs, but not always related to symptoms
- Ultrasound or MRI: Used in chronic or unclear cases to assess fascia thickening or tearing
Treatment
Conservative Options:
- Stretching exercises (plantar fascia and calf muscles)
- Footwear modifications and heel cushions or orthotics
- Ice and NSAIDs
- Physical therapy: Manual therapy, taping, and modalities
- Night splints to maintain a stretch overnight
Interventional Options:
- Corticosteroid injections
- Shockwave therapy or platelet-rich plasma (PRP) in refractory cases
Surgical Options:
- Surgery is rarely needed
