Achilles Tendinopathy
What Is It?
Achilles tendinopathy refers to degeneration or inflammation of the Achilles tendon, which connects the calf muscles to the heel bone. It may be insertional (at the heel) or mid-portion (above the heel). It’s often caused by overuse, especially from repetitive running, jumping, or sudden increases in activity.
Symptoms
- Pain or stiffness along the back of the heel, especially in the morning
- Swelling or thickening of the tendon
- Tenderness that worsens with activity
- Possible bump or nodular thickening along the tendon
- Pain with calf stretches or heel raises
Diagnosis
- Physical exam: Palpation, range of motion testing, and calf squeeze tests
- Ultrasound or MRI: Used to assess tendon degeneration, tears, or inflammation
- X-rays: May show calcification at the tendon insertion (in insertional tendinopathy)
Treatment
Conservative Options:
- Rest and activity modification
- Eccentric strengthening exercises
- NSAIDs for symptom relief
- Heel lifts or orthotics to reduce tendon strain
- Physical therapy: Soft tissue mobilization, stretching, modalities
Interventional Options:
- Shockwave therapy or PRP injections for chronic cases
Surgical Options:
- Surgery in cases of rupture or persistent degeneration
