Gluteal Tendinitis
What Is It?
Gluteal tendinitis involves inflammation or degeneration of the gluteus medius and minimus tendons at their attachment near the greater trochanter. These muscles are critical for pelvic stability during walking and single-leg stance.
Symptoms
- Lateral hip pain worsened with activity
- Tenderness over the greater trochanter
- Night pain when lying on the affected side
- Difficulty with walking, stair climbing, or single-leg stance
- Weakness with hip abduction in advanced cases
Diagnosis
- Physical exam with resisted hip abduction and gait analysis
- Ultrasound to assess tendon thickening or tears
- MRI for detailed tendon and soft tissue evaluation
Treatment
Conservative Options:
- Activity modification and load management
- Physical therapy focusing on hip abductor and core strengthening
- NSAIDs and ice therapy
Interventional Options:
- Corticosteroid or PRP injections for persistent symptoms
Surgical Options:
- Rarely indicated, reserved for refractory cases with tendon tears
