Patellofemoral Pain Syndrome (Runner’s Knee)

What Is It?

Patellofemoral pain syndrome involves irritation of the cartilage and soft tissues under and around the kneecap.

It is commonly related to overuse, maltracking of the patella, muscle imbalance, or biomechanical issues.

Symptoms

  • Dull, aching pain at the front of the knee
  • Pain worsened by stairs, squatting, kneeling, or prolonged sitting
  • Popping or clicking sensations

Diagnosis

  • Clinical exam with pain reproduced by patellar compression or resisted knee extension
  • X-rays to assess alignment
  • MRI in select cases to evaluate cartilage or soft tissue pathology

Treatment

  • Activity modification and avoidance of aggravating movements
  • Physical therapy focusing on quadriceps, hip, and core strengthening
  • Taping techniques or patellar tracking braces
  • Ice and NSAIDs
  • Foot orthotics to address biomechanical contributors
