Lumbar Facet Syndrome
What Is It?
Lumbar facet syndrome involves degenerative or arthritic changes of the facet joints, which are the small joints located at the back of the spine.
These joints guide spinal motion, and degeneration can lead to localized mechanical low back pain without significant nerve involvement.
Symptoms
- Aching low back pain, sometimes radiating into the buttocks or thighs
- Pain worsened with spinal extension, twisting, or prolonged standing
- Morning stiffness or stiffness after inactivity
- Pain often improves with sitting or spinal flexion
Diagnosis
- Physical exam with palpation of facet joints
- X-rays or CT scans demonstrating joint degeneration
- Diagnostic medial branch block to confirm facet-mediated pain
Treatment

- Physical therapy focusing on core stabilization and posture
- Manual therapy including mobilization or manipulation
