Lumbar Spinal Stenosis
What Is It?
Lumbar spinal stenosis is narrowing of the spinal canal or nerve passageways (foramina) in the lower back.
This narrowing compresses the spinal nerves and is most often caused by degenerative changes such as arthritis, disc bulging, or thickened ligaments.
The condition commonly develops gradually with aging.
Symptoms
- Lower back and leg pain with standing or walking
- Symptom relief when sitting or leaning forward
- Numbness, tingling, or weakness in the legs
- Sensation of heaviness or fatigue in the legs
- Activity-related symptoms that improve with rest (neurogenic claudication)
- Physical exam and gait or walking tolerance testing
- MRI to confirm central canal or foraminal narrowing
- CT myelogram if MRI is contraindicated
- X-rays to assess degenerative changes and alignment
Treatment
Conservative Options:
- Activity modification and posture correction
- Physical therapy for strength, flexibility, and endurance
- NSAIDs or neuropathic pain medications
Interventional Options:
- Epidural steroid injections for symptom relief
Surgical Options:
- Laminectomy or decompression for significant or progressive symptoms
