De Quervain’s Tenosynovitis

What Is It?

De Quervain’s tenosynovitis is inflammation of the tendons within the first dorsal compartment of the wrist.

It typically affects the abductor pollicis longus and extensor pollicis brevis tendons and is associated with repetitive thumb or wrist motion.

Symptoms

  • Pain and swelling near the base of the thumb
  • Pain with gripping, pinching, or lifting
  • Catching or snapping sensation with thumb movement
  • Positive Finkelstein’s test

Diagnosis

  • Clinical diagnosis based on history and physical exam
  • Ultrasound or MRI if the diagnosis is unclear

Treatment

Conservative Options:

  • Thumb spica splinting
  • NSAIDs and ice therapy

Interventional Options:

  • Corticosteroid injection into the tendon sheath
  • Shockwave therapy or PRP

Surgical Options:

  • Surgical release for chronic or resistant cases
